Parent Checklist
A completed registration consists of fees paid in full and submission of all required documents, plus a mandatory fundraiser equal to $200. League-supplied helmet and shoulder pads require a $200 deposit. Players may not participate in practice or games until all items are met.
All documents must be turned in before July 1st.
Required Documents

| Birth Certificate | Copy of certified county birth certificate or passport. No baptism or hospital certificates. The original is needed at certification. |
| Physical | Must be on the official Pop Warner physical form (provided by the league). In color — no photocopies of the form accepted. |
| Report Card | Must show the child's entire year's grades. |
| Utility Bill | Most current utility bill or bank statement for proof of address. |
Monetary Responsibilities
